How to become an NDIS participant with Blue Sky Health: 

​

  1. Complete our online NDIS form (Click Here). 

  2. We will contact you for further information (e.g. Service Agreement) and to organise your initial session. 

  3. Your first session will be with our NDIS Performance Manager, who assists you to identify your goals and objectives. 

  4. Once you are happy with the goals and objectives the Performance Manager saves them onto our online software and emails through your unique online login details to the system so you can view the goals and objectives as well as session notes and any alerts (e.g. incidents). 

  5. Follow up sessions will be scheduled with a Therapeutic Support Staff (e.g. Psychologist) or a Behaviour Support Practitioner. 

  6. We will contact you throughout the course of your therapy with us to ensure you are happy with the progress/assist you with any problems/questions or concerns you might have about our services.

Goals and Objectives: 

At Blue Sky Health all NDIS participants and anyone they allocate (e.g. Support Coordinator/ parents/ guardian etc) will receive their unique login details to our online system, where they can view the participant’s goals and objectives as well as session notes and any alerts (e.g. incidents).

In providing supports or services to people with disability, a person covered by the Code must:

​

  • Act with respect for individual rights to freedom of expression, self-determination and decision-making in accordance with applicable laws and conventions

  • Respect the privacy of people with disability 

  • Provide supports and services in a safe and competent manner, with care and skill

  • Act with integrity, honesty and transparency

  • Promptly take steps to raise and act on concerns about matters that may impact the quality and safety of supports and services provided to people with disability

  • Take all reasonable steps to prevent and respond to all forms of violence against, and exploitation, neglect and abuse of, people with disability

  • Take all reasonable steps to prevent and respond to sexual misconduct.

The National Disability Insurance Scheme (NDIS) provides funding directly to people with disability. At Blue Sky Health our NDIS services are designed based on the NDIS code of conduct which can be found HERE:

One of the services that some people with disability require is a Behaviour Support Plan also known as BSP or a Positive Behaviour Support Plan. Blue Sky Health is a registered NDIS Provider in order to assist NDIS participants regarding Behaviour Support and Therapeutic Support Services. We can also assist regarding Restrictive Practices (RP). For more information please visit:
